Problem Cards
Whether you get the right answer or not is not the only thing relevant to how often the cards should be reviewed in lessons. I wonder if we might make it so that the frequency of review is also affected by how long you spend on that card. Obviously, if it takes you a split second to review one card, but you have to sit there for fifty seconds recalling the answer to the prompt on another card, you know the first one a lot better than the second.

Advanced algorithms
Our view on this is that it could make the app more difficult to anticipate, and confuse some users. Plus, there are lots of different algorithms that you could think of, based on all sorts of things, like time, and it is not clear which should be used.
We have decided to leave it all up to the user. If you find yourself having to think hard about an answer, you can always mark it 'wrong' yourself. The user is complete control of what is considered wrong. It might just be that you don't think you knew the answer 'well enough'.
